This page provides access to the Fee-for-Service (FFS) Pharmacy Prior Authorization Request Forms.

Pharmacy prior authorizations must be initiated by a Medicaid prescriber. Please select the appropriate form below and fax the completed form, along with chart notes and any other required documentation, to the Pharmacy Prior Authorization Team at (855) 828-4992.
