Preferred Drug List

Utah Medicaid’s Preferred Drug List (PDL) was established under SB 42, passed by the Utah Legislature in 2007. SB 87 further authorized Utah Medicaid to require prior authorization for non-preferred drugs, effective May 18, 2009.

The Utah Medicaid Preferred Drug List (PDL) and Pharmacy Resource Document is updated monthly and includes information on prior authorization requirements, product limits, over-the-counter products, brand-over-generic requirements, and the 3-month supply list. The PDL is not a comprehensive list of all Utah Medicaid-covered outpatient medications. For additional coverage information, please refer to the Utah Medicaid Pharmacy Provider Manual.
